Revise Signed Documentation

The following describes how to revise clinical documentation for prior visits that have already been electronically signed.

Finding the Documentation for a Visit

If the documentation to be revised is from a prior date, you find the visit using the Clients tab in documentation. A past visit that is signed will be indicated by the check mark. To view the documentation for that date of service, select it from the list.

You will need to unsign the visit documentation before revising it. To do so, click Unsign. You will be prompted to enter a reason and your password to unlock the documentation, as illustrated.

Once you have revised your note, you must Sign the documentation again.

Note: A snapshot of your documentation content, before and after the documentation revision, is kept in the database for future reference. The revision reason is stored and displayed in audit reports used by management.
