Add Diagnosis Codes before Signing Off

You must enter at least one diagnosis code in the Case Information tab of the Client Editor for each payer associated with the chart note. If you have not entered the necessary diagnosis codes, a warning will display, based on payer requirements.

If the payer associated with your case is a non-HIPAA covered entity that has elected not to migrate to ICD-10 diagnosis codes, you will receive the following warning.

You can add an ICD-9 diagnosis code by performing the following steps.

  1. Click Add to insert a new line in the ICD-9 Diagnosis tab.
  2. Either click to display an alphabetical list of ICD codes, or click to display the Search for Diagnosis dialog and search for an ICD code either by code or description.
  3. Click Add and Close to add the selected ICD-10 code and return to the Chart tab.
  4. Click Sign to sign off the chart note.

If the payer associated with your case is a HIPAA covered entity that has migrated to ICD-10 diagnosis codes, you will receive the following warning.

  1. Click Add to insert a new line in the ICD-10 Diagnoses tab.
  2. Either click to display an alphabetical list of ICD codes, or click the to display the Search for Diagnosis dialog and search for an ICD code either by code or description.
  3. Click Add and Close to add the selected ICD-9 code and return to the Chart tab.
  4. Click Sign to sign off the chart note.

If there are two payers associated with your case and one is a HIPAA covered entity, while the other is not and has elected not to migrate to ICD-10 diagnosis codes, you will receive the following warning.

You can add an ICD-9 and ICD-10 diagnosis code by performing the following steps.

  1. Click the ICD-9 Diagnoses tab.
  2. Click Add to insert a new line in the tab.
  3. Either click to display an alphabetic list of ICD codes or click to display the Search for Diagnosis dialog and search for an ICD code either by code or description.
  4. Click Add to add the selected ICD-9 code.
  5. Click the ICD-10 Diagnoses tab.
  6. Click Add to insert a new line in the tab.
  7. Either click the arrow to display an alphabetic list of ICD codes or click the ellipsis to display the Search for Diagnosis dialog and search for an ICD code either by code or description.
  8. Click Add and Close to add the selected ICD-10 code and return to the Chart tab.
  9. Click Sign to sign off the chart note.
