Set Up Documentation Cosigning

In this topic, you will learn how to set up your clinic to enable cosigning of clinical documentation.

Cosign Global Setting

The Cosign feature allows management to decide whether or not all daily chart notes are signed off by the Supervising Therapist, or just the evaluations.

Follow these steps to define the cosign global setting:

  1. From the Settings menu, select Company and Clinic Info. The Company Info and Clinic Management dialog box appears.
  2. In the Global Settings section of the dialog box, select the Require cosign for all documentation by non-supervising therapist check box in order to require a supervising therapist to cosign all clinical documentation completed by a therapist or assistant who requires cosignature.

If the Require cosign for all documentation by non-supervising therapist is check box is selected, all daily documentation will be routed to the Supervising therapist for review and cosign. If this check box is unselected, only evaluation and re-evaluation documentation will be routed to the supervising therapist for a cosignature.
  3. Click OK to save the setting.

Note: It is your responsibility to take into account all pertinent regulations and payer requirements to determine whether or not the supervising therapist must cosign all documentation or just evaluations.

Regardless of this global setting, the supervising therapist’s credentials will be listed as the rendering provider on all claims if the May treat without supervisor cosign check box is unselected in the staff editor for the treating clinician.

Clinical Supervisor Settings

In the Staff Editor dialog box, clinicians must be classified as to whether or not they are allowed to treat patients without supervision. This has important implications for claim processing and cosigning clinical documentation.
